Patient Information

If you are a new patient, you should receive a new patient packet in the mail to complete. Bring the completed packet, your current insurance card(s) along with your co-payment. You will be requested to present your driver’s license or a government issued photo ID with you at the time of your appointment.

Bring copies of recent lab test results, x-ray reports, etc.

Please bring a list of all your medications. You may want to bring all medication bottles as well.

 

Contact the Business Office at (859) 278-2232, ext. 100 before your initial visit to confirm insurance coverage if this was not done at the time your appointment was scheduled or if insurance coverage has changed since your appointment was made.


PRESCRIPTION REFILLS

 

Please call your pharmacy where your original prescription was filled to request a refill of your medication. Your pharmacist will contact KDEC to request any pertinent information. Allow up to 2 business days to have the prescription authorized.

We do not call patients to advise them we have filled a prescription.

Check directly with your pharmacy to verify your medication has been filled.

Prior Authorizations of medications directed by your insurance coverage can take up to a week and longer if appeal letters are requested.


BILLING AND CREDIT POLICY

 

Depending on your contract with your insurance company, you may be required to make a co-payment or pay a deductible amount. Co-payments are due at the time of service. Failure to pay your co-payment at the time of service may result in a billing fee. We accept cash, checks and credit and debit cards.

If you do not cancel your appointment or if you fail to come in to your appointment you will be charged a $25 fee.

Insurance payments paid directly to KYDEC will be applied to your account. You will receive a billing statement if there is a balance remaining. All charges are due and payable upon receipt.
