If you are a new patient, you should have been asked when scheduling if you wanted to receive a new patient packet by mail or complete it via the website. For a downloadable version, choose from one of the options below. If you elected mail, you should be receiving information shortly.

Please call your pharmacy where your original prescription was filled to request a refill of your medication. Your pharmacist will contact KDEC to request any pertinent information. Allow up to 2 business days to have the prescription authorized. We do not call patients to advise them we have filled a prescription. You will be contacted if there is an issue filling your prescription. Check directly with your pharmacy to verify your medication has been filled. Prior Authorizations of medications directed by your insurance coverage can take up to a week and longer if appeal letters are requested.
Depending on your contract with your insurance company, you may be required to make a co-payment or pay a deductible amount. Co-payments are due at the time of service. Failure to pay your co-payment at the time of service will result in an additional billing fee. We accept cash, checks and credit and debit cards.
Insurance payments paid directly to KYDEC will be applied to your account. You will receive a billing statement if there is a balance remaining. All charges are due and payable upon receipt. To view our complete financial policy, click here.
